Consists of records relating to the gathering of information for the purpose of course and teaching evaluation. Records may include student opinion survey or questionnaire responses, summaries and correspondence.

Until operational use ceases
Retention based on anticipated operational use.
Signed student responses may be placed in Tenure and Promotion files. Student responses may be returned to course director.
