Consists of records relating to clients who are provided counselling services concerning personal, academic or career concerns by clinicians. Records may include intake information, notes made by service providers in order to provide counselling care, referral letters, consent forms and related documentation.

Retention Rationale
Retention is based on expected operational need and current practice. When the client is younger than 18 years old, the retention of 10 years begins after the client's 18th birthday.
